I don't know if this is related, but I had UT 2004 stop working at one point. It was all because I had changed a volume setting while playing the game. The driver for the audio that I had been using (RealTek High Definition) got messed up because I did this. So I updated it and it was fine. In conclusion, check all drivers.

Sometimes one of your ini files will get corrupted on a bad shutdown or a crash. Try deleting UT2004.ini and User.ini (back up first) and start UT.
