1. Time and date of the incident.
2. Name of the player being reported.
3. Which rules you feel were violated.
4. If the violations were conducted in text chat, voice chat, in the forums, or in-game.
5. If the violation was not in text chat, the names of any other players who may have observed the incident.
6. Your player name.
7. Any relevant screenshots or video (optional, but helpful.)
