You may want to contact your server provider for support. They will be able to verify the query port is correct and it is open in the firewall.

I would recommend they try to test scanning on HLSW entering 208.75.229.42:10495:10496

They will be able to see that it is not reporting any data.

Hi,

It's SWAT4 server not Half-Life based engine one, and all required ports has been opened on a server.
Since Gamespy shut down their servers SWAT4 during startup is not opening query port (it can't connect to main server). The only way at the moment is to run that game in LAN mode with query port handled by GEZ mod plugin.
It reports game state and all required info about it, but it's not 100% compatible with Gamespy query.
It can be tested with 3rd party game browser created by DExUS - you can add your server without problems using query port (game port+1).
