Find and click the name of the player in the online list or the history. On the player stats, fine the "Player Profile" section and click the link below it to link your account to it.

You can do that on the Manage page http://www.gametracker.com/server_info/89.40.233.144:27015/manage/
If no options are available, you need to create a team on http://www.gametracker.com/teams/ first.
