I've moved all 11 of my TF2 servers to a new host and thus had to change IPs. Using the manage tab on GT I added the DNS to all of my servers before I moved them and they seemed to switch over and keep their GT ranking. Except one; my Zombie Mod server, which is ranked #2 for TF2 servers. Any reason why it didn't transfer over like the others?

Post the IP, port of both the new and original server.

But,
If the new server is on the site already, you need to claim it before it will transfer over, per the instructions.

Old Server: 74.91.123.72:27015
New Server: 198.27.120.29:27015

I'm not sure if it is on the new site because this is the only server that has had this problem out of the 11 total we transferred.

As you can see, the new server was added prematurely before the DNS moved it over:

You'll need to claim the new page before it will move over.

Ah. Must've been from when we had CS:GO servers on that range as well... I've claimed both servers, do I just wait now for it to update 192.27.120.29 (the new server IP) to the old IPs rank?

It should do so within 24 hours after the DNS is pointed to the new IP.

Hasn't switched yet, am I able to manually merge the two pages and keep the old rank?
