There is only one way to apply to have a server unbanned, or to rejoin the rankings;

Your server has not been deranked by Gametracker.
You have deranked it yourself. There is no 'undo' for this action, and nothing Gametracker can do either. Your rank will have to restore naturally. This normally takes no more than a week.
