My Garry's Mod server is not properly tracking the number of players online or the current map when I claim the server. If I unclaim the server and leave it, it will track the players and the current map. http://www.gametracker.com/server_info/104.128.51.152:27045/

Claiming the server on GameTracker does not have any impact on how your server responds to the query. I suggest downloading a utility like HLSW http://hlsw.org/ and comparing what you see on GT to what you see on HLSW when the problem is happening.
