I resolved the issue. Went directly to the source.

Your server is not reporting player scores, so unfortunately we cannot fudge scores to place next to player names. You can confirm this for your self on a tool such as HLSW (hlsw.org).
Please review any server settings you changed that caused this or review issues with mod authors if relevant.
