You should be able to do this from the manage area for the server page. If it is a GT Premium or GameServers.com server, the button may be disabled. If you are not able to delete it yourself, you'll need to wait for the website to automatically remove it after extended inactivity. Since this is your old server, you can just stop visiting the page and it will be removed in the near future.
