I've tried to troubleshoot this for at least a week now with my host:

But unfortunately we haven't been able to get the server name recognized at all.
It doesn't recognize the map or the server name, this is becoming really frustrating and I am not sure what else to do at this point.
Any help would be very much appreciated!

I have updated your query port to 25565 which it should be per what you stated in the thread. This has resolved the issue.
